Service Category Claim Form

Please complete this form when once the face to face booking has been completed.

Prior to conducting interviews, Program Co-ordinators will inform you of your eligibility for Service category C,D or E at the time of booking.

Your form will be processed within 5 business days.

Important: There is a 90-day deadline from booked interview date for submitting invoicing for additional payments in connection with approved claims. After this 90 day period the additional payments will be voided and can no longer be claimed.

We request that service providers invoice for these additional payments in the same invoice as when invoicing for the base rates of $60 remote and $70 In Person.

Please only use the Service Category Claim Form for bookings that have been approved by the Program Co-ordinator. Service Category Claim forms submitted without prior approval will not be actioned.
